Please Bring the Following Items to Your First Appointment:
- New Patient Forms
You can find our new patients forms here to print and fill out prior to your first appointment, saving time with your check in process. If you are a new patient, we ask that you arrive 20 – 30 minutes prior to your scheduled appointment to make sure all registration paperwork is processed. - Personal ID & Medical Insurance Cards
Please bring your photo ID and insurance cards each time you visit the office. You can conveniently find a list of Payment Options and Insurance Plans that we accept in the links provided, or give us a call with any questions. - Past Medical Information: X-Rays, Test Results & Medications
In order to provide the best care possible and create the optimal treatment plan, please bring along any medications, previous test results, diagnostic films and x-rays available to you for your first visit.
You can call our main office location at (414) 488-1818 to request an appointment or use our online Request an Appointment form. If you receive our voicemail, we will contact you within 24 hours to schedule your appointment.
At Dagam Neurosurgery, our goal is to provide timely care to those needing appointments. We understand though that situations arise where you must cancel your appointment, however, we require a minimum of 24 hours notice. View our official Appointment Policy here.
